Teacher Wish List Application

This form is for Hayward School District teachers to express specific needs for classrooms, teams, or boosters. Requests made must be for students and not teacher’s personal use. Items donated will become the property of the Hayward Community School District and not that of the requestor.

Please note that This is not a HFEE Grant. The grants are separate from the Wish List Items. Once all the forms are completed then you can expect a 2-3 week turnaround time for posting. Once your request has been funded, please consider a $10 donation to HFEE to help support the administrative costs associated with this wish list portal. Please visit our funded requests page once your request has been funded. HFEE will need to update the site accordingly so that requests are not over-funded.
